SBIRT/NARCAN Program 

Provoking Hope works in partnership with the first-responders of Yamhill County to assist in emergency situations involving possible overdoses. 

Crisis referral between first- responders and Provoking Hope is designed to meet a client’s specific needs. Through mentoring and support, Provoking Hope strives to provide for the good of the client. Our goal is the  safety, well-being, and facilitation of reunification to move toward permanent recovery.

IMPACT TEAM 

Provoking Hope is partnered with other agencies in our county to help transition our homeless community into stable housing situations. 

We have a team that is set-up to reach out to the homeless and show them that there is another way. We supply them with hygiene kits. We let them know that they can come in and meet with a mentor who will set them up with potential resources that can assist them in their journey to change their situation. Most importantly, we show them lots of love and compassion.

HOPE HOUSE #1 

Hope House is a venture that Provoking Hope is beginning with the mission to provide safe, clean, managed, Stage 1 to Stage 2 Recovery Living and Transitioning into a life of recovery and lifestyle living. We are very excited. Hope House #1 is for ladies only, no children.

Youth Outreach Recovery Services

This program is designed to target ages 14 to 22-year-old youth and to provide a safe and sober environment and team for youth and their families. ARC

Adult Recovery Court Program

Certified Recovery Mentors (CRM) provide Adult Drug Court (ARC) participants with day to day support and encouragement.

CRM’s assist with accessing resources, finding sober living, and advocating for the participant. We also work with coordination of care with partnering agencies to support participants with seeking services for mental health, chemical dependency, employment, and addressing each individual’s needs.

GAMBLING SUPPORT PROGRAM

Provoking Hope facilitates a new program to support those struggling with the addiction of Gambling.

Gambling Addiction, is also known as compulsive gambling or gambling disorder. Compulsive gamblers can’t control the impulse to gamble, even when it’s hurting themselves or their loved ones.

TTRS

Transitional Treatment Recovery Services 

The TTRS Program is for Men and Women w/Children who may or may not be involved with DHS. There are 3 Women’s Homes, 1 Men’s Home, 1 Family Home and a 30-day Stabilization Home. The program is an 8 to 12 month or longer treatment program and includes 4 Phases with a Transformation Phase at completion.

The residents are required to be engaged in Chemical Dependency at Yamhill County Adult Behavioral Health as well as Mental Health and other services such as Parenting with Lutheran Community Services and Recovery Work with CRM’s.

Certified Recovery Mentors work within the homes to help walk the recovery path with each client. CRM’s engage with clients for weekly 1on1’s; transports to Groups, Doctors appointments, and Recovery meetings.

Certified Recovery Mentors are recovering addicts who are working an active recovery program and want to help give back to our community.

JMAT

Jail Medically Assisted Treatment Program

Provoking Hope is now partnering with the Yamhill County Correctional Facility on the launch of a pilot program for medication assisted treatment (MAT) for inmates with opioid addictions such as heroin. Yamhill County Sheriff’s Office has received funding from Oregon Health Authority to administer a MAT program while individuals with opioid use disorder are in custody. Inmates with a history of using heroin or other opioids who qualify for the program are given the opportunity to start Suboxone, a medication that dramatically reduces cravings. Suboxone is an FDA-approved medication for assisted detox of people with opioid addiction.

 

To ensure the individual remains in treatment after leaving the facility, Provoking Hope’s JMAT Certified Recovery Mentor will be going into the jail and meeting with individuals in the MAT program to mentor them, help set them up for after care appointments, and help transition them onto a successful pathway of recovery.

DHS PROGRAM

Provoking Hope works in partnership with DHS to ensure the well-being of children in addicted families and the potential reunification of those families.

Certified Recovery Mentors provide wrap-around support to parents who struggle with addiction. Our mentors help these parents embrace not only a clean lifestyle, but a whole new way of life. Through modeled behavior and shared experience, these partnerships foster sustained safety and well-being of families as a whole.

Jail In-Reach Program

Provoking Hope is now sending Certified Recovery Mentors into the Yamhill County Jail. Our CRM have the lived experience to connect with inmates like no one else can. Here are a few ways we provide support to those in need

IOP

Intensive Outpatient Program

 

Providing peer delivered recovery support services to individuals in Intensive Outpatient services through Yamhill County Adult Behavioral Health. CRM’s work in collaboration with clinicians to provide integrated care. CRM’s work with their peers individually and attend IOP groups.

Our mentors work with clients of IOP to develop and achieve recovery goals. CRM’s provide support and encouragement in navigating systems, developing life skills and building connections to the recovery community. Mentor’s used lived experience to provide empathy and compassion to the peers they serve.
